Include My Information In the Directory

If you want to be included in the School Directory, click the "Yes" button and enter your information below. If you do not want to be included, enter your child's name and teacher name and click the "No" button, so that we will know that you do not want to be included.

Family Info

If your family members have different last names, enter both the first and last names in each field.
In the "last name" field enter the name you want used for your family listing in the directory.

When entering phone numbers, indicate whether they are (h)ome, (m)obile or (w)ork.
For "Primary Phone" enter the one phone number that you want printed in the classroom section of the directory.
